Harm Reduction Counselor
Iris House, a nonprofit agency in west Harlem is seeking full-time Harm Reduction Counselor for our Harm Reduction Department. She/he Counselor will be responsible for conducting individual and group counseling on topics including sexual risk reduction, IDU risk reduction, HIV and/or Hepatitis C secondary prevention and medical treatment plan adherence.
Position includes but not limited to the following;
Conducts individual, group, and/or family counseling.
Ensures clients understand and respect the confidentiality of treatment.
Assists patients in negotiating and understanding the various policies and procedures of the clinic (i.e. fees, annual physicals).
Monitors and assesses patient progress, develops engagement plans, and periodic evaluation as specified in state regulations.
Coordinates treatment within agency.
Makes appropriate referrals to outside agencies; assists patients in transactions with legal, social services and other agencies.
Performs essential data gathering activities.
Update profiles as necessary noting transactions and matters relevant to treatment especially those areas outlined by the supervisor and/or in program procedures.
Completes all paperwork in a professional and timely manner.
Secures any approvals, releases, etc., required for treatment, as stated in the program policy and procedures manual.
Attends various community meetings and makes presentations relative to services and program requirements.
Advises community of needed volunteer services and recruits individuals and participates in all required staff meetings.
Diagnose clients and makes outside referrals when appropriate.
Advise and orients prospective clients of program policies and clients rights, and the particular responsibilities of the client.
Make any necessary appointments required during intake process.
Responsible for arranging appropriate coverage of caseload during all scheduled absences and to notify supervisor of same in a timely manner.
REQUIREMENTS:
Bachelor’s degree required and one year’s experience or equivalent. CASAC or CASAC-T and two years experience or equivalent. Excellent verbal and written communication and organizational skills. Knowledge of special needs of individuals living with HIV/AIDS. Bilingual (Spanish/English) preferred.
