Iris House provides case management services to clients using a team based model of service delivery for people who are HIV+ and Medicaid eligible. We provide advocacy and support for our clients with such pressing issues as:
- Housing
- Financial/Entitlement
- Child Care
- Job Placement Assistance
- Medical Services
- Drug Treatment
- Education
- Referrals
- Acupuncture/massage
- Nutritional Services
We work hard to help our clients accomplish their goals through frequent assessments, accompaniment to medical and legal appointments as needed and one-on-one sessions.
